Ngāti Whātua Ōrākei

Health Insurance Enrolment

Complete all sections of this form to enrol into the Ngāti Whātua Ōrākei Health Insurance. To be eligible, you must be a registered Ngāti Whātua Ōrākei hapū member living in New Zealand. Anyone 18 and over must complete a separate form.

The registration process occurs once annually. Applications are processed during April-May and your policy will start on 4 July, with the welcome pack received in June.

Registered and eligible Ngāti Whātua Ōrākei hapū members will be covered under the Ngāti Whātua Ōrākei Health Insurance. Any additional cover for yourself or any cover for non-member whānau will be at your own cost. For any assistance, free phone the friendly nib team on 0800 nib nwo (0800 642 696).

To be a part of the Ngāti Whātua Ōrākei Health Insurance and to complete the enrolment process, we need your consent for:
  • nib to share some of your personal information with Ngāti Whātua Ōrākei and its related entities(e.g. your name, contact details, high level details about the claims you have made, the level of engagement with the kaiarahi and your frequency of claims, any suspicious activity and if there is a cancellation of your policy) to enable specific Ngāti Whātua Ōrākei personnel to encourage engagement with the kaiarahi, refer you to relevant programs, ensure you are making the most out of your cover, and any other related purposes as agreed between nib and Ngāti Whātua Ōrākei.
  • Ngāti Whātua Ōrākei to share some of your information with nib (e.g. your name and contact details) to ensure your information is up to date and for nib to share communications with you.
If you do not provide your consent, we cannot enrol you in the Ngāti Whātua Ōrākei Health Insurance.

Knowing your privacy rights
You have rights under the Privacy Act regarding your private information. At any time, you can access, check and correct, or ask for a copy of any information, nib or Ngāti Whātua Ōrākei have about you.
